چکیده
A 58-year-old woman presented with severe abdominal pain. Laboratory findings showed markedly raised amylase and lipase concentrations at 1169IU/L and 5040IU/L respectively. She denied any alcohol consumption. Triglyceride and calcium levels were within the normal range. An abdominal computed tomography (CT) scan showed a round, 2.7-cm, low density, mass-like lesionwith a central area of air density in the second part of the duodenum (●" Fig.1). Esophagogastroduodenoscopy (EGD) revealed a bezoar impacted in a juxtapapillary duodenal diverticulum (JPDD;●" Fig.2). The hard phytobezoar was broken up and removed using grasping forceps. After the bezoarhadbeen removed, her pancreatitis improved. A subsequent endoscopic retrograde cholangiopancreatography (ERCP) showed that the JPDD was located caudally to the papilla with the papillary orifice hardly visible, being located deep inside on the ceiling of the diverticulum. No biliary sludge or stones were noted. A sphincterotomy was performed to prevent recurrence of the pancreatitis (●" Fig.3). JPDDs are usually located on the cephalic side of the major papilla and may be associated with biliary stasis or relapsing pancreatitis [1–4]. Food or bezoar impaction in a JPDD is known to cause biliary obstruction; however, pancreatitis due to bezoar impaction in a JPDD has never been reported, although there are reports of pancreatitis due to impaction of a large bezoar in the duodenal lumen itself [5]. The reason for this seems to be the anatomical proximity of the common type of JPDD to the bile duct rather than the pancreatic duct (●" Fig.4). In this report, bezoar impaction in a rare caudally located type of JPDD caused acute pancreatitis. This is a rare cause of acute pancreatitis and an unusual clinical manifestation of a JPDD caused by its location.

Recurrent pancreatitis caused by a huge intraluminal duodenal diverticulum.
A 34-year-old man presented with abdominal pain radiating to the back. He reported several similar episodes in the preceeding years and consecutively had lost 22 pounds in weight. The patient denied alcohol abuse. Laboratory investigations suggested biliary pancreatitis (lipase 4580 U/L, bilirubin 2.4 mg/dl, γ-GT 317 U/L). Physical examination was unremarkable except for moderate epigastric ten...
متن کاملJuxtapapillary Duodenal Diverticula Risk Development and Recurrence of Biliary Stone
We assessed whether the presence of juxtapapillary duodenal diverticula (JPDD) risks biliary stone disease and recurrence. In total, 695 patients who underwent ERCP were divided into two groups: biliary stone disease (group I, n = 523) and non-stone biliary diseases (group II, n = 172). Additionally, for a control group (group III), 80 age-matched healthy subjects underwent side-view duodenosco...
متن کاملCT features of juxtapapillary duodenal diverticula with complications.
OBJECTIVE We evaluated the computed tomography (CT) features of juxtapapillary duodenal diverticula (JPDD) with complications in patients who had acute abdomen. MATERIALS AND METHODS Nineteen JPDD were evaluated in 14 patients (mean age: 50 years), who had acute abdomen on contrast-enhanced helical CT with a diagnosis of complicated JPDD by endoscopic retrograde cholangiopancreatography (ERCP...
متن کاملJuxta-ampullary intraluminal diverticulum and acute pancreatitis.
CONTEXT Acute pancreatitis is usually due to well-known causes, such as biliary lithiasis and alcohol consumption. Anatomic abnormalities may represent a less frequent but important etiological factor. CASE REPORT The case of a 27 year old women complaining of acute pancreatitis associated with a large duodenal juxta-papillary diverticulum is presented. CONCLUSIONS Anatomic causes of pancre...
متن کاملA rare cause of biliary obstruction and pancreatitis
The prevalence of duodenal diverticula varies considerably in the literature, between 3-24% [1,2]. They are acquired extraluminal outpouchings of the mucosal wall through the muscularis propria and their incidence increases with age. While most duodenal diverticula do not cause major symptoms, they are increasingly recognized as causes of pancreaticobiliary disease. Food bezoar in a juxtapapill...
